Hazelden's outpatient addiction treatment programs provide real solutions to help men, women, youth, and families faced with the disease of addiction. We understand that addiction is complicated. Our team of clinical professionals has the experience and expertise to treat the whole person--by addressing the physical, mental health, and spiritual aspects of addiction.

Hazelden is an in-network provider for Aetna, Blue Cross Blue Shield of Minnesota, Patient Choice, Value Options, Health Partners, and Preferred One.
Intensive Outpatient Program for Adults
- Offers a treatment model that allows for individuals to fully experience treatment and incorporate it with their lives, whether working, volunteering, caring for family, or going to school.
- Intended for those who do not require medically supervised detoxification or stabilization, and have stable mental and physical health.
- Program meets for 12 group hours per week under the same addiction treatment principles as residential programs.
- Individual sessions with chemical dependency counselors.
- Next step is typically the Extended Outpatient Program.
Extended Outpatient Program for Adults
- Offers less intensive treatment model that focuses on living in emotional balance and concentrating on Steps 4-9.
- Intended as a continuation from other levels of care, individuals must have completed a prior level of treatment that focused on Steps 1-3.
- Continuing Care Group meetings.
Intensive Outpatient Program for Adolescents
- Offers a treatment model allowing adolescents to participate in programming while attending school and extra curricular activities as well as social and family activities.
- Intended for those who do not need a medically supervised detoxification or stabilization and have stable mental and physical health.
- Ages 12 and above.
- Participants must be clinically appropriate based on a chemical health assessment.
- Program meets for 12 group hours per week with one individualized counseling session.
Assessment Services
Hazelden offers multiple drug and alcohol use assessment services either in person, by phone, or on the web.
- In-person, chemical use assessments with a licensed mental health practitioner to determine whether treatment services are recommended and if so, what type.
- Court-ordered assessment. A more lengthy evaluation is also available for those who have been court-ordered to have an assessment done or for those that need assessment documentation.
- Phone-based chemical use assessment with a licensed alcohol and drug counselor to determine whether Hazelden treatment services are recommended and if so, what level of care.
- Free, self-guided, online alcohol and drug screening tool at aboutmydrinking.org for assessment, prevention, education, and intervention.
- Assessment costs vary and a fee schedule can be obtained at any location.
Mental Health Services
- Recovery-sensitive outpatient mental health care for adults, couples, and families. Outpatient mental health services are available to anyone affected by addiction, including family members, friends, and anyone in the community.
- Integrated clinical care at Hazelden (the simultaneous treatment of addiction and a co-occurring mental health disorder) addresses all aspects of health and self — body, mind, and spirit.
