Substance abuse addiction treatment for adolescents and young adults
Plymouth, MN
Since 1981 Hazelden has helped thousands of adolescents and young adults find freedom from alcohol and other drugs and regain hope for healthier and more fulfilling lives.

Situated on 15 picturesque, wooded acres along the north shore of Medicine Lake in Plymouth, Minnesota, our youth campus provides the ideal setting to begin the journey to recovery.
Hazelden's recovery programs are designed to meet the unique needs of each young person. We know that teens and young adults are most open to change when treated in an environment of dignity and respect. Our unique approach has led to our recognition as a national leader in the treatment of teen alcohol and drug addiction.
Programs and services
Assessments and evaluations
At Hazelden, we do not assume a diagnosis of alcoholism or other addiction exists when people contact us for help. Our expert alcohol and drug assessment process provides an accurate diagnosis along with recommendations for follow-up care. Evaluations are done via phone, outpatient program, or residential stay -- depending on the needs of the individual. Whatever the form, an assessment is the place to start.
Residential treatment
Hazelden's addiction treatment programs are based on years of research and study. Our philosophy is to treat each person who comes to us with respect and dignity while providing a safe and comfortable place in which to heal. Our programs include integrated mental health services for co-occurring mental health issues. Each patient's length of stay is truly individualized and based on the client's needs. Males and females are provided care on separate gender-specific units.
Because of its intensive nature, the course of residential treatment may range from 2-26 weeks based on individual clinical needs.
Outpatient care
Hazelden's outpatient treatment programs work under the same principles as the residential programs and provide individualized focus within the program structure. Treatment is abstinence based and uses the evidence-based Twelve Step approach. Clients receive alcohol and drug assessments that are used to develop their individualized treatment plan. The clinical team then determines whether a mental health assessment is needed. Hazelden Plymouth has a fully staffed Rule 29 mental health clinic on-site to help address co-occurring disorders, assessments, individual therapy, or medication evaluation.
The intensive outpatient program at Hazelden's Plymouth campus serves those who do not require medically supervised detoxification or stabilization, have stable mental and physical health, have had few or no prior addiction treatment experiences, and have a supportive home and work or school environment.
Family programs
Recovery from addiction is stronger when family members are involved. For the family members of patients at our adolescent facility, we offer two programs:
Mental Health Services
- Recovery-sensitive outpatient mental health care for youths and their families. Outpatient mental health services are available to anyone affected by addiction, including family members, friends, and anyone in the community.
- Integrated clinical care at Hazelden (the simultaneous treatment of addiction and a co-occurring mental health disorder) addresses all aspects of health and self — body, mind, and spirit.
- Mental health and chemical health assessments
- Individual psychotherapy
- Couples and family therapy
- Stress management
- Coping skills
