How to give
Hand delivery of stock certificates
Certificates may be given to a Hazelden Development Officer or delivered to the
Hazelden Office of Development at our main headquarters in Center City, MN.
NOTE: Please DO NOT mail endorsed certificates. Once the certificate is endorsed, it is legal tender and can be exchanged by anyone.
It is strongly recommended that certificates not be sent to a transfer agent for registering in Hazelden's name. This procedure is unnecessary and delays the transfer.
You will need to give the following delivery information to your broker to make a gift to Hazelden:
