You choose how to give . . . Below are the most common ways to make a gift. If you don't see the information you need, feel free to contact the Development Office at giving@hazelden.org or 888-535-9485. Online Have your credit card ready and donate online today. Cash/Checks If you prefer to make a gift to Hazelden by cash or check, simply complete the mail-in form and enclose it with your gift. Checks should be made payable to the Hazelden Foundation. Please include any specific fund designation or call the Development Office for more information. Credit Card To make a donation by credit card, the Development office needs your name, the name printed on the card, your address, the credit card number, expiration date and the amount of your donation. We will also need the area or designation of your gift. You can choose to either donate online, by phone 888-535-9485, by fax (651-213-4553) or by mail. Sustainer Pledge Sign up for automatic monthly or quarterly electronic donations through our Sustainer Pledge program. Stock and Securities For information regarding the valuation of stocks and bonds or gift crediting, contact Hazelden's Gift Reporting Specialist at 651-213-4295. The most efficient method of giving stocks and bonds is by electronic transfer. Wire Money Transfer Wire transfers have historically been used to transfer funds between financial institutions when time was of the essence. More recently, the use of wire transfers has fallen by the wayside as the fees incurred by the donor have become quite substantial. While Hazelden will gladly accept wire transfers, we suggest that when our donors are trying to meet a deadline (i.e. December 31) they either charge their gift to a credit card or "overnight" the donation for next day delivery.
