Referents
How to start an admission
Call the Hazelden Resource Center at 800-257-7810, or the specific
admission number of the facility you wish to refer to. A pre-intake specialist will begin the process by collecting basic information and history and will arrange for the client or referent to speak to an intake counselor.
Families may also speak to a financial counselor regarding funding. As a referent the clinical case manager or lead counselor will become your contact person as the admission progresses and throughout a patient's stay. How long it takes to complete the admission will vary from program to program.
Hazelden also has clinical outreach representatives who work with referents in different regions of the country. Call 800-257-7810 to request a call back from the outreach representative in your area. They can answer a host of questions applicable to all programs within the Hazelden system. If you wish your client to call directly please advise them to include your name and contact information as the referring professional.
