How may I order a transcript?
You may download a transcript form (above) and submit it by one of the following methods:

• By Mail:  Hazelden Graduate School of Addiction Studies, Attn: Registrar, 15251 Pleasant Valley Road-CO9, P.O. Box 11, Center City, MN 55012
  
• By FAX: 651-213-4710 (only if paying by credit card)
  
• In Person: At the Graduate School, Cork Center, Center City, MN

Transcript requests cannot be accepted by telephone or email.

Is there a charge?
Each official transcript is $5.00. Payment is required at the time of the request. There is no charge for unofficial transcripts (student copy). 

How do I pay?
Payments can be made by credit card, check, or cash. Faxed requests will only be accepted using credit card payment.

How long does it take to process my request?
Please allow approximately 1-7 business days for processing.

May I request my transcript now and receive it after my grades are posted?
Yes. Please indicate at the time of the request that you wish the transcript sent after the current semester grades are posted.

May I request my transcript now and receive it after my degree is posted?
Yes. Please indicate at the time of the request that you wish the transcript sent once the degree has been posted.
