Driving

From Portland/Beaverton:

• Go south on I-5 to Tualatin/Sherwood Exit 289
• Turn right (west) onto Nyberg Road

From Portland International Airport

• Go south on I-205 approximately 24 miles to I-5 north
• Go north on I-5 to Tualatin/Sherwood Exit 289
• Turn left (west) onto Nyberg Road

Then:

• Go approximately .2 mile to Tualatin/Sherwood Road. Stay left and follow main thoroughfare.
• Go 4.5 miles to Hwy 99/Pacific Highway west. Turn left.
• Go 7.7 miles to Villa Road. Turn right.
• Go one mile to Mountain View Road. Turn right.
• Go .2 mile to Esther Street. Turn right.
• Go .2 mile to Hazelden. Turn right into parking lot.

From Salem:

• Go north on I-5 to Exit 278, Donald/Aurora
• Turn left (west) onto Ehlen Road, which becomes McKay
• Go 6.8 miles to Hwy 219. Turn right toward Newberg
• Stay on 219 into Newberg where it will bend to the right and become N. Villa Rd. In one block there is a light (get in far right lane and go straight through the intersection)
• N. Villa Rd. turns into Villa Rd. after intersection
• Go one mile to Mountain View Rd. Turn right.
• Go .2 mile to Esther Street. Turn right.
• Go .2 mile to Hazelden facility. Turn right into parking lot.

Patients should book your flight to Portland International Airport (PDX) and call us in advance with the name of your airline, your flight number, and estimated time of arrival. We will arrange to have you picked up at the airport by Frontier Transportation/NW Limousine. 

Family program participants are responsible for arranging their own transportation and lodging.

From the arrival gate, go downstairs to baggage claim and obtain your luggage.  Then go outside of the airport and use the crosswalk to get to the other side of the roadway. When you have reached the other side, turn right and go to Reserved Vehicle Area 5 (note this is not Baggage Carousel 5 inside the airport). Your driver will be standing next to a black Suburban or Lincoln Town car and will be holding a sign bearing your first name, last initial -- if you do not appear at the expected time, the driver will page you inside the airport. The driver cannot leave the vehicle.
